HomeMy WebLinkAboutEDB Proposed Amendment No 1 to SA No 4 with Dewberry-Hydro for Governors Park EXECUTIVE SUMMARY AGENDA ITEM: Proposed Amendment No. 1 to Supplemental Agreement No. 4 with Dewberry-Hydro, for Civil and Environmental Engineering Professional Services Agreement 2018/2019 RFQ No. 3 to provide Professional Engineering Services for Detailed Design and Services During Construction Date: June 3,2022 BACKGROUND: Clay County Utility Authority (CCUA) staff requests approval from the Board of Supervisors to execute Amendment No. 1 to the above referenced Supplemental Agreement,under which the consultant completed the preliminary design and master plan. This amendment serves as a continuation of SA4 to complete detailed design,bidding, and construction administration. CCUA received requests for utility service to developments around the Governor's Park Development of Regional Impact (DRI). CCUA previously acquired land at the Governor's Park Site to serve the utility needs of regional developments when constructed. To meet the anticipated service schedules of the -0a proposed developments,CCUA must begin designing the potable water treatment and distribution facilities. aai Engineering Services for the design and construction of sewer collection and treatment facilities to serve ca the area were awarded under a separate agreement. 0 E Supplemental Agreement No. 4 (SA4) previously authorized Dewberry-Hydro to conduct preliminary design, and site master planning through buildout for potable water supply facilities on this site. SA4 also m Ce included geotechnical, environmental, and well development services in anticipation of the need for Phase 1 design. CCUA Staff determined the best path forward to meet the immediate utility needs is to design and construct an interim facility using a combination of new equipment and equipment that CCUA currently owns but is not in service. A more traditional water distribution facility will follow. Many of the elements installed for use in the interim facility will continue to be useful in future phases of the plant. CCUA requested the attached scope and fee proposal from Dewberry-Hydro to prepare detailed design plans and bid specifications for the interim plant,rehabilitation of existing equipment,and pre-purchase of equipment with long lead times.The proposal also includes permitting,project management,and inspection services as well as the development of a second well. Dewberry-Hydro will coordinate with Ardurra,who is designing wastewater pumping and reclaimed storage and distribution facilities for this site. This amendment is achieving efficiencies by segregating common site activities such as surveying, environmental permitting, and stormwater design under the Ardurra contract and limiting the water plant scope of work to activities specific to the water plant. Amending SA4 authorizing Dewberry-Hydro to perform these functions will provide project continuity and make efficient use of information gathered and produced under the original Supplemental Agreement. BUDGET: Projected expenditures for the design and construction of the interim Governor's Park WTP are within the CIP budget. The current Capital Improvement Plan(CIP)includes sufficient funding to cover this project. CCNA: CCNA, F.S. s. 287.055, does apply to the use of a consultant under continuing contract. The anticipated construction cost for the project does not exceed$4 million. Staff negotiated the Supplemental agreement in compliance with F.S.s.287.055.Staff and the consultant negotiated the scope and fee for the professional services at a compensation that CCUA determines is fair, competitive,and reasonable. RECOMMENDATION: Staff respectfully recommends the Board of Supervisors approve the attached Amendment No. 1 to Supplemental Agreement No.4 with Dewberry-Hydro to provide detailed design,bidding,and construction administration services for the Governor's Park Water Plant in the amount of$742,700.00. Project Background and Description Dewberry I Hydro (CONSULTANT) was requested by Clay County Utility Authority (AUTHORITY) to provide a preliminary design and master plan for Governor's Park WTP (GPWTP). This project was on a fast track to facilitate completion of construction of the first phase by July 2023. In the development of the preliminary design report for the facility, it was determined that the construction cost of the initial permanent facility, to accommodate projected flows through 2028, would exceed the $4 million threshold above which the AUTHORITY is required to competitively solicit engineering services for the design and construction of the facility. It was also determined that a new"in service" date for the permanent water supply and treatment facility would be in December 2024. The AUTHORITY needs to have an interim water supply and treatment facility in service by September 2023, to meet contractual obligations to supply water for nearby development. Because of the accelerated timeline for meeting this obligation, discussion with the AUTHORITY to indicates that this interim facility will utilize surplus equipment and tankage from decommissioned AUTHORITY water treatment facilities to the extent possible, to avoid fabrication and delivery delays and to minimize additional costs for the interim treatment facility. The interim facility will provide water from September 2023 to the in-service date for the permanent facility in December 2024. The estimated average daily flow (ADF) for the interim facility for this period is 0.387 million gallons per day (MGD), based on the construction of 667 ERCs at 580 gallons per day (GPD) per ERC. The unit ERC rate includes allowance for irrigation, since reclaimed water will not be available during this period. If the interim facility is to serve up to 800 ERCs, the estimated ADF for the interim facility for this period is 0.464 MGD. The range of ADFs affects only the required storage on site, as discussed below. The AUTHORITY has existing equipment (pumps, horizontal ground storage tanks, aerators, etc.) at the Postmaster's, Peter's Creek, Lucy Branch and Greenwood water treatment plants that it wishes to have evaluated for incorporation into the first phase of design and construction for the GPWTP. The interim facility is anticipated to include the following major components: • Two 16-inch water production wells (one operating and one standby)with new well pumping equipment with capacity of 1,770 gallons per minute (GPM) for each well. Each will be driven by a constant speed motor and equipped with a flow meter. These wells will serve the permanent water treatment facility also. These wells will be constructed under a separate construction contract from the interim water treatment plant. Page 1 of 14 • An existing surplus tower aerator at Lucy Branch WTP that will be relocated to the site and reused until the permanent facility is in service. • Existing surplus steel horizontal ground storage tanks (at Greenwood, Postmaster's and Peter's Creek WTPs) that will be relocated to the site with a total capacity of 126,000 gallons. These tanks are sufficient for the 667 ERCs. Should the ERCs exceed 667 and range up to 800 ERCs, additional storage will be required to a total of 135,000 gallons, which may be added if needed through the addition of another steel storage tank or construction of the first of the permanent storage tanks. • Existing surplus main and jockey high service pumps (at Lucy Branch and Greenwood WTPs) will be rebuilt and relocated to the site. These pumps currently do not have sufficient pump head to service the Governor's Park development. Three 1,400 GPM pumps will be rebuilt with new impellers, 100 HP motors and require new VFDs. The total high service pumping capacity required is estimated 2,100 GPM (667 ERCs) or 2225 GPM (800 ERCs). In addition, one existing 400 GPM jockey pump will be rebuilt with new motor and VFD to provide sufficient head for the service area. These pumps will be rehabilitated by the local equipment representatives for the pump manufacturers. It is anticipated that for the interim facility, the pumps will be located on an outdoor slab to meet the in-service schedule. These pumps will be available to be used in the permanent facility at which time the outdoor slab will have a building constructed over it to as part of the permanent facility. • A temporary sodium hypochlorite storage and metering facility will be required. • A new hydropneumatic tank with air compressors will be required, to buffer the pump capacities for the anticipated low flows as the new development homes are added. • Electrical and instrumentation will be required. A new electrical service from the utility will be required and will be sized for the permanent facility. Standby power will be provided by an existing surplus standby generator relocated to the site. A new fuel storage tank will be required. Because of the accelerated schedule and supply chain issues, the following procurement approach will be required: • The CONSULTANT shall prepare separate rehabilitation specifications for the rehabilitation of the existing high service and jockey pumps and the engine-generator set, for the AUTHORITY's use in procuring the required modifications/repairs with the local vendors that service that equipment. • The CONSULTANT shall identify and prepare a pre-purchase equipment package for the AUTHORITY to solicit bids and pre-purchase equipment that has long-lead times, including selected electrical equipment (e.g., motor control centers, variable frequency drives) and the well pumping equipment. • The CONSULTANT shall prepare a contractor bid package for all other work, including the installation of the pre-purchased and locally rehabilitated equipment and pumps. Dewberry HYDRO Page 2 of 14 Note that the specialty well construction work will be bid directly to well drilling contractors under a separate construction contract. In addition to the need for the interim water treatment facility, other modifications to the previously authorized task assignment have been identified, as follows: • The original scope identified the design, bidding and construction of one potable water well, using the existing consumptive use permit for Governor's Park. Analysis conducted indicates that two potable water wells will be required to provide the required redundancy. In addition, the well casing size will be increased from 12-inches to 16- inches to accommodate the required flow rate from each well. Additional aquifer performance testing will be required to meet permit conditions and the consumptive use permit will require modification for the relocation and higher water withdrawal. CONSULTANT shall provide professional engineering services for the design, permitting, bidding and construction services for the interim water treatment facility, and additional services associated with the construction of an additional well, as outlined in the scope of services. B. Scope of Services Upon authorization to proceed from the AUTHORITY, the CONSULTANT will provide the to following identified services. This authorization shall be in full force and effect until the CONSULTANT completes all services as described in the CONSULTANT Supplemental Agreement and any subsequent modifications hereto. PHASE 100 — PROJECT MANAGEMENT Task 101 — Project Management This task consists of overall management of the Project including contract administration, budget management, invoicing, monthly status reports, project scheduling, and coordination with the AUTHORITY and CONSULTANT's sub-consultants. Task 102 — Meetings The CONSULTANT will attend meetings as tabulated below with the AUTHORITY. Other specific meetings are included in the tasks described in this scope of services. The CONSULTANT will prepare a meeting agenda and meeting minutes. Meeting minutes shall be distributed within three working days of the date of the meeting. 1. Kick-off meeting. 2. Site visits to evaluate specific existing equipment identified for the interim water treatment plant at Lucy Branch, Greenwood, Peter's Creek, and Postmaster's WTPs. Dewberry HYDRO Page 3 of 14 3. 60% and 90% design review meetings for the interim WTP design. The 60% review meeting will include the electrical/I&C designer. 4. Monthly progress meetings over the course of the 16-month schedule. The scope assumes eight (8) meetings will be in person and eight (8) meetings will be virtual via Teams. PHASE 200—SUPPORT SERVICES Task 201 — Land Surveying Services Land surveying services will be provided under an existing consultancy contract with the AUTHORITY's consultant, which is performing other design work at the Governor's Park site. The land survey will be provided to the CONSULTANT for use in designing the facilities of this contract. No separate fee is included for surveying in this scope. Task 202 — Existing Tank Evaluation The tank inspection services shall be provided in accordance with the scope of services set forth in a May 8, 2022, proposal prepared by SUBCONSULTANT Tank Rehab, Inc. to The project proposes to use existing horizontal steel ground storage tanks (GSTs) that have been decommissioned and relocate them from their current sites at Greenwood, Postmaster's, and Peter's Creek WTPs, as follows: • Greenwood: 50,000 gallons • Peter's Creek: 27,000 gallons • Postmaster's: 28,000 gallons and 21,000 gallons The GST at Greenwood WTP was inspected in 2020 and found to be in good condition. The other tanks have not been recently inspected. CONSULTANT will use a tank inspection subconsultant to inspect the GSTs at Peter's Creek and Postmaster's WTPs to verify the required rehabilitation and suitability for use at the interim facility. Inspections will be done on a unit cost basis (cost per tank), with an allowance of four inspections. Should one of the identified tanks not be suitable for use, the fourth inspection will be utilized for another surplus tank. Task 203 — Geotechnical Engineering Services The geotechnical services shall be provided in accordance with the scope of services set forth in a September 10, 2021, proposal prepared by SUBCONSULTANT Meskel &Associates Engineering. These services have been previously authorized and are not included in the budget for this amendment. Dewberry HYDRO Page 4 of 14 Task 204—Additional Hydrogeological Services Hydrogeological services for the design and construction of one new water supply well for the project were included in previous authorizations. As identified under Project Background, additional hydrogeological services are required for the construction of an additional new well, modeling associated with modification of the Consumptive Use Permit (CUP), preparation of a permit modification and additional aquifer testing to meet permit conditions. Additional well construction services will be provided per the scope of services set forth in a May 3, 2022, proposal prepared by SUBCONSULANT ASRus, LLC. Ground water modeling in support of a permit modification to the existing CUP will be provided per the scope of services set forth in the May 6, 2022, proposal prepared by SUBCONSULTANT Liquid Solutions Group, Inc. This modeling is provided as a cost not-to- exceed contingent service that will be utilized only if authorized by the AUTHORITY. Well Construction Administration Services to CONSULTANT will provide the following construction administrative services during the construction of the Governor's Park production wells: • Prepare and distribute a pre-construction booklet. • Conduct a pre-construction meeting with the drilling contractor and CCUA. • Review project submittals related to the well construction program. • Provide technical interpretations and evaluate deviations from the design and specifications. • Coordinate and attend six (6) monthly progress meetings to discuss well construction- related issues. • Prepare progress meeting minutes and agenda. • Review and prepare responses to up six (6) requests for additional information (RFI). • Prepare and distribute weekly summary reports. • Provide continuous consulting to the AUTHORITY. CONSULTANT will be responsible for analyzing the geophysical and video logs to generally assess aquifer properties and well construction conformance with the specifications of the new production wells. Prepare Consumptive Use Permit Modification CONSULTANT will submit a permit modification to the SJRWMD of CCUA's CUP #137335-2 to reflect two Upper and Lower Floridan Aquifer production wells at Governor's Park WTF. This Dewberry HYDRO Page 5 of 14 includes attending a pre-application meeting with the SJRWMD, preparing the application forms, supporting documentation, tables, figures, and appendices. The groundwater flow modeling will be performed by Liquid Solutions Group, Inc. to support the permit modification effort, including groundwater flow model simulations and summary documentation of results. CONSULTANT will prepare up to two (2) copies of a draft CUP permit application package for submission to CCUA. After the draft permit has been reviewed, CONSULTANT will submit two (2) copies of the CUP permit modification package to the SJRWMD and two (2) copies of the CUP permit application CCUA for their records. Preparation of the CUP permit modification is provided as contingent services that will be utilized only if authorized by the AUTHORITY. This task includes two (2) responses to any request for additional information by the SJRWMD. PHASE 300 — REHABILITATION AND PRE-PURCHASE DOCUMENTS Task 301 — Rehabilitation Documents As soon as practicable in the 60% design, the CONSULTANT shall prepare specifications for the rehabilitation of the following equipment: to • Three (3) 1,400 GPM high service pumps currently located at Lucy Branch WTP. These Aurora pumps will require new impellers for higher head and motors. This existing Aurora pumping equipment will be rehabilitated at the facility of the Aurora pump representative that serves the AUTHORITY. • One (1) 400 GPM jockey pump currently located at Greenwood WTP. This Peerless pump will require a new impeller for higher head and motor. This existing Peerless pumping equipment will be rehabilitated at the facility of the Aurora pump representative that serves the AUTHORITY. • One (1) 400 kW Caterpillar model 3406 diesel generator located at the Fleming Island WTP. This existing generator will be rehabilitated at the local facility that serves the AUTHORITY. Task 302 — Pre-Purchase Documents As soon as practicable in the 60% design, the CONSULTANT shall prepare specifications for the pre-purchase of long-lead equipment, to include major electrical equipment and the well pumping equipment. Pre-purchase specifications will be provided to the AUTHORITY for soliciting equipment bidding and procurement. 11 Dewberry HYDRO Page 6 of 14 PHASE 400 — FINAL DESIGN DOCUMENTS Task 401 — 60% Design Documents CONSULTANT will prepare and submit 60% design documents, consisting of the following: 1. Design drawings, consisting of three (3) half-size set of drawings to scale, and one (1) electronic copy in PDF format. These drawings shall be prepared as 22-inch by 34-inch full-sized drawings and distributed as 11-inch by 17-inch half-size sets to scale. 2. Technical specifications, consisting of three (3) bound copies and one (1) electronic copy in PDF format. 3. One (1) copy of a preliminary engineer's opinion of probable construction cost (EOPCC). The anticipated sheets are listed in Attachment A-2. Task 402 — 90% Design Documents to CONSULTANT will prepare and submit 90% design documents, consisting of the following: 1. One (1) copy of a tabular summary of CONSULTANT's responses to comments provided by the AUTHORITY for the 60% design submittal. 2. Design drawings, consisting of three (3) half-size set of drawings to scale, and one (1) electronic copy in PDF format. These drawings shall be prepared as 22-inch by 34-inch full-sized drawings and distributed as 11-inch by 17-inch half-size sets to scale. 3. Technical specifications, consisting of three (3) bound copies and one (1) electronic copy in PDF format. The technical specifications shall include Division 1. 4. One (1) copy of an updated EOPCC. If the EOPCC varies by more than 25% from the 60% design EOPCC, CONSULTANT shall provide the rationale for the difference in cost. Task 403 — 100% Design Documents CONSULTANT will prepare and submit 100% design documents, consisting of the following: 1. One (1) copy of a tabular summary of CONSULTANT's responses to comments provided by the AUTHORITY for the 90% design submittal. 2. Design drawings, consisting of three (3) half-size set of drawings to scale, and one (1) electronic copy in PDF format. These drawings shall be prepared as 22-inch by Dewberry HYDRO Page 7 of 14 34-inch full-sized drawings and distributed as 11-inch by 17-inch half-size sets to scale. 3. Technical specifications, consisting of three (3) bound copies and one (1) electronic copy in PDF format. 4. One (1) copy of a final EOPCC. PHASE 500 — PERMITTING SERVICES This phase consists of the preparation and submittal of the forms and documents that are required for obtaining regulatory and construction permits/approvals for the Project. The costs of the permits are not included in the fee and will be paid by the AUTHORITY. Task 501 — Specific Permit to Construct Public Water System (PWS) Components This task consists of the preparation of the forms and supporting documents that are required for the AUTHORITY to obtain regulatory and construction permits/approvals. CONSULTANT shall prepare for and attend one (1) pre-application meeting. The scope for the Specific Permit to Construct PWS Components shall include the following to services: 1. Prepare and submit permit application (including all required forms and supporting documentation) to the Florida Department of Environmental Protection (FDEP). 2. Prepare and submit responses to Requests for Additional Information (RAls). This effort is limited to two (2) sets of review questions/comments unless otherwise required to address errors and/or omissions of CONSULTANT in preparing and submitting requisite application documents. Task 502 — Environmental Permitting Environmental permitting services will be provided under an existing consultancy contract with the AUTHORITY's consultant who is performing other design work at the Governor's Park site. The environmental permits will include facilities designed under this scope of services. The CONSULTANT shall coordinate with and provide information to the AUTHORITY's consultant as required for the environmental permitting. No separate fee is included for environmental permitting in this scope. Task 503 — Clay County Development Review CONSULTANT shall prepare and submit a Development Review Package to Clay County. It is assumed that the site will not require re-zoning. Dewberry HYDRO Page 8 of 14 CONSULTANT shall prepare for and attend one (1) Development Review Committee pre- submittal meeting. CONSULTANT shall prepare the Development Review Package. CONSULTANT shall prepare and submit up to two (2) RAIs. All permitting fees are to be paid by the AUTHORITY. Zoning changes for the parcel are not included in this scope of services. PHASE 600 — BID PHASE SERVICES This phase will be performed during the bid phase of the Project. It is assumed that the AUTHORITY will enter into one (1) contract for construction of the Project. The AUTHORITY shall be responsible for preparation of Division 0, (Procurement and Contracting Requirements) advertisement of the Project, and distribution of the Project documents, including all costs associated therewith. Task 601 — Bid Documents CONSULTANT shall prepare and submit the following bid documents to the AUTHORITY: 1. One (1) half-size (11"x17") set of construction plans, one (1) electronically signed to and sealed half-size (11"x17") PDF set, and one full-sized (22"x34") set. 2. One (1) copy of technical specifications, and one (1) electronically signed and sealed PDF set. 3. Schedule of Values and Bid Sheet (electronic only). 4. Scope of Work (separated out for advertisement). 5. Supplementary conditions (electronic only). 6. Engineer's Opinion of Probable Construction Cost. Task 602 — Pre-Bid Meeting CONSULTANT shall attend one (1) pre-bid meeting, including a pre-bid site visit (if required). Task 603 — Bid Addenda CONSULTANT shall prepare bid addenda as required to address/respond to questions and comments submitted to the AUTHORITY by prospective bidders. Task 604— Bid Recommendation CONSULTANT shall evaluate bids, including qualifications of contractors, and recommend award. Dewberry HYDRO Page 9 of 14 PHASE 700 — CONSTRUCTION ADMINISTRATION SERVICES These services will be performed during construction of the Project. For the purpose of this Amendment, it is assumed that the construction duration will not exceed 360 calendar days. Task 701 — Pre-Construction Meeting The CONSULTANT, including the electrical and l&C designer, shall attend one (1) pre- construction meeting and prepare a meeting agenda and meeting summary. Task 702 — Preliminary Matters and Contract Documents The CONSULTANT shall provide one (1) signed and sealed set of conformed construction drawings (1 full-size set, one half-size set and one electronic copy, in PDF format, of conformed construction drawings) and technical specifications (one (1) hard copy and one (1) electronic copy, in PDF format)for distribution to the AUTHORITY. The CONSULTANT shall provide the contractor the required number of signed and sealed documents in accordance with the bid documents. The conformed construction drawings and technical specifications shall incorporate all changes and clarifications to the documents during the bid and pre-award phases. to The CONSULTANT shall review and approve the preliminary schedule of shop drawings and submittals, the preliminary schedule of values, and the construction schedule for the Project. The CONSULTANT shall provide written interpretations and clarifications of the Contract Documents as requested by the contractor and determine the acceptability of work. The CONSULTANT will render in writing their opinions concerning the contractor's requests for formal decisions on claims and disputes. The CONSULTANT will disapprove or reject work which, in the opinion of CONSULTANT, is defective and/or requires special inspection or testing as may be deemed necessary to assess conformance with the Contract Documents. Task 703 — Shop Drawing Review Review contractor's shop drawings and other submittals for general conformance with the Contract Documents. The CONSULTANT shall conduct up to eighty (80) reviews of shop drawings, including initial and re-reviews. Task 704— Construction Progress Meetings and Factory Acceptance Testing (FAT) Attend twelve (12) monthly construction progress meetings and prepare a meeting agenda and meeting summary (unless meeting agenda/minutes are included in contractor's scope). The CONSULTANT will attend one (1) FAT at a local integrator's factory, for acceptance testing of control system panels and components before shipment to the project site. The testing Dewberry HYDRO Page 10 of 14 procedures shall be as provided by the Contractor or their Subcontractors. The CONSULTANT will provide testing minutes, completed forms based on Contractor's submittal, and a listing of any deficiencies to be corrected before equipment shipment. Task 705— Construction Site Visits The CONSULTANT's Engineer of Record (EOR), or designated representative, will visit the site at intervals deemed appropriate by the CONSULTANT to review the construction of the Project for conformance with Contract Documents. The project budget includes two (2) visits a month for a seven-month active construction on site. Task 706— Limited Progress Inspections The CONSULTANT shall provide limited construction phase services. The purpose of this work is to have sufficient knowledge of the construction to certify the project complete to the regulatory agencies and the AUTHORITY. The CONSULTANT shall assist the AUTHORITY during the construction. The CONSULTANT shall also perform inspections of the Project on a regular basis to provide information on the Contract Documents and document that the work is being performed in accordance with the intent of the design. The CONSULTANT will witness or to review test results and require corrective action for failing tests. Full-time inspection of the work being performed is not required. It is anticipated that the construction inspector be required for thirty-one (31) weeks of the 52-week construction period for sixteen (16) hours per week; however, this may be subject to modification during if additional on-site activities require CONSULTANT to be present. Task 707 —Applications for Payment The CONSULTANT will review Applications for Payment submitted by the contractor, including updated progress schedules, and will indicate in writing to the AUTHORITY a recommendation of payment or return the application to the contractor specifying in writing the reasons for not recommending payment. The CONSULTANT's recommendation of payment will constitute a representation by the CONSULTANT to the AUTHORITY that: 1. The work has progressed to the point indicated; 2. The quality of the work, as observed by the CONSULTANT, is within the design intent of the Contract Documents; and 3. The conditions precedent to the contractor's being entitled to such payment appear to have been fulfilled based on the CONSULTANT'S site observations. In the event that the work has not progressed in accordance with the approved construction schedule, the CONSULTANT will request that the contractor submit a schedule recovery plan indicating corrective actions necessary to put the work back on schedule. The CONSULTANT Dewberry® HYDRO Page 11 of 14 shall submit the contractor's Application for Payment to the AUTHORITY within five (5) working days of receipt of the same. The CONSULTANT will review up to twelve (12) pay applications. Task 708 — Requests for Information (RFIs) Review RFIs submitted by the contractor and provide written responses to the contractor. The CONSULTANT shall maintain an RFI log for the Project. It is anticipated that CONSULTANT will review up to thirty-eight (38) RFIs. Task 709 — Change Orders/Contingency Releases/Allowance Authorization Releases (AARs) The CONSULTANT will review requests from the contractor for changes in Contract Price or Contract Time and provide written comments to the contractor and the AUTHORITY. If acceptable, the CONSULTANT will prepare a written Change Order to be signed by the CONSULTANT and the contractor and submitted to the AUTHORITY for approval. This task includes the review of Allowance Authorization Releases (AARs) and contingency releases. The CONSULTANT shall review up to six (6) change orders or contingency/AAR. Task 710 — Substantial Completion to Upon written notification and receipt of a list of items to be completed/corrected from the contractor, the CONSULTANT will visit the site to determine if the work or a designated portion thereof is substantially complete. For the purpose of this task, the CONSULTANT assumes that two (2) substantial completion site visits will be required. When the work (or a portion thereof) is deemed to be substantially complete, the CONSULTANT shall prepare a Certificate of Substantial Completion that establishes the date of Substantial Completion. The Certificate of Substantial Completion shall be accompanied by a list of items to be completed/corrected. Task 711 — Project Close-Out Upon written notification from the contractor that the entire work is complete, the CONSULTANT shall visit the site to determine if the work is complete as required by the Contract Documents. For the purpose of this task, the CONSULTANT assumes that one (1) final completion site visit will be required. When the work is deemed to be complete by the CONSULTANT and the AUTHORITY, the CONSULTANT shall review the contractor's final Application for Payment and accompanying documentation and provide a written recommendation of payment to the AUTHORITY. The CONSULTANT shall also give written notice to the AUTHORITY and the contractor that the work is acceptable subject to the provisions of the AUTHORITY'S General Conditions. The CONSULTANT shall review contractor's as-built drawings for completeness and compliance with AUTHORITY requirements. This effort shall include the preparation of written 11 Dewberry HYDRO Page 12 of 14 comments for submission to the contractor based on the CONSULTANT's review of the as-built drawings. The CONSULTANT shall prepare record drawings based on the contractor's as-built data. One (1) complete 22" x 34" set of draft record drawings shall be submitted for review by the AUTHORITY. The CONSULTANT shall incorporate all comments from the draft review, then submit signed and sealed electronic (PDF) set and AutoCAD files and one (1) signed and sealed 22" x 34" hard copy set of the record drawings to the AUTHORITY within 30 days of the receipt of the contractor's final as-built drawings and AUTHORITY comments. The CONSULTANT shall prepare and submit a "Certification and Request for Clearance to Place Permitted PWS Components into Operation" (Form 62-555.900(9)) to FDEP for each non- contiguous portion of the Project. The CONSULTANT shall prepare and submit an "As-Built Certification and Request for Conversion to Operation Phase" (Form 62-330.310(1)) to the SJRWMD. C. Deliverables The CONSULTANT shall prepare and submit to the Authority, including electronic format when applicable, deliverables as identified in the tasks of this scope of services. to D. Schedule The CONSULTANT will proceed with the services identified herein immediately upon receipt of an executed copy of this Consultant Purchase Order and a formal Notice-to-Proceed (NTP) from the AUTHORITY. The projected schedule will have milestones to ensure completion by September 2023 for the completion of construction and clearance for operation of the interim phase of the water plant. A preliminary version of the Project schedule in Microsoft Project is included as Attachment A-3. E. Compensation This AGREEMENT establishes a lump sum cost of$742,700.00. Basic services are $707,610.00. Optional services, as described under Task 204, are $35,090.00. Optional services require prior authorization from the AUTHORITY. Compensation for the services performed under this Agreement shall include all personnel services, any reimbursable expenses, which may include, but are not necessarily limited to, such items as reproduction, travel, communication expenses, and postage and shipping. Attachment A-1 shows the breakdown by task. F. The AUTHORITY's Responsibilities The AUTHORITY will provide the following information for the CONSULTANT and /or perform the following services related to the Project: Dewberry® HYDRO Page 13 of 14 • Allow CONSULTANT scheduled access to the project site. • Provide one representative by The AUTHORITY to coordinate, schedule, and receive comments/information related to this project. • The AUTHORITY will furnish the CONSULTANT with readily available studies, reports, and other data pertinent to the CONSULTANT's services. All documents and data provided by the AUTHORITY shall be provided in digital format (e.g., PDF, database files, spreadsheet, etc.) to the greatest extent feasible and practical. G. Services Not Included Service not specifically described in the tasks above are not included. H. Assumptions The Scope of Services and compensation arrangement outlined in this Consultant Purchase Order are based on the following assumptions: to 1. Project water quality is assumed to require no additional treatment beyond cascade tray aerator and disinfection. 2.
