BACKGROUND:
BHA completed design of the CCUA Administration Building Expansion Phase 1, and CCUA
received bids from contractors on May 27, 2022. KBT Contracting Corporation submitted the
lowest responsive bid at a total lump sum of$4,600,705.
Staff asked BHA to provide a scope and fee to provide construction administration services for the
project. Staff reviewed the attached proposal and considered the fee to be reasonable for the
services provided.

Staff respectfully recommends the Board of Supervisors' approval of Supplemental Agreement
No. 7 engaging BHA in the amount of$93,071.60 for construction administration services for
the CCUA Administration Building Expansion Phase 1 project.

Item Description of Services
Provide Construction Administration Services as
requested and identified in the attached scope and fee
proposal dated June 28, 2022.
Construction Administration Services $92,014.10
Permitting Fees $1,057.50
Total Lump Sum Price $93,071.60
The Contract time shall commence on the needed date. This document along with the June 28, 2022, Fee
Proposal shall become an amendment to the existing agreement for Architectural and Engineering Design
and Permitting Services 2017/2018 RFQ No. 1. All provisions, terms and conditions of the existing
Agreement will apply to this Supplemental Agreement.

Dear Ms. Shoemaker,
Bhide & Hall Architects, P.A., is pleased to present this proposal letter for Professional
Construction Administrative Services for the proposed New Administration Building and Site
Improvements project at 3176 Old Jennings Road, in Middleburg, Florida. Our proposal is
based off our design services proposal dated July 28, 2021 and approved on August 4, 2021.
Scope
• Construction Administrative Services to include:
o Shop Drawing review
to o Construction submittal review
o Response to contractor generated Requests for Information (RFI)
o Attendance at routine (OAC) Owner, Architect, Contractor progress
meetings held either on site or remotely. We include attendance at said
meetings on a bi-weekly basis, for a maximum of 20 meetings. Additional
meetings can be attended remotely via conference call or video-call.
o Attendance at scheduled routine inspections for design conformance.
o Final inspection for substantial completion with punch list and follow up
inspection for conformance to design.
o Review of contractor provided close-out documentation for conformance
with contract requirements.
o Delivery of Record drawings to owner at completion of project.

Construction Administration Fee
We propose a lump sum Construction Administration Services Fee based on the Total
Construction Cost at award. The contractor, KBT Contracting Corp., was awarded this project
at a construction cost of$4,600,705.00.
CA Services (Percentage of Estimated Construction Cost—2%): $92,014.10
Permitting Fees: $1,057.50
Total: $93,071.60
