Ratification of Executive Director actions for the Emergency Purchase of recovery,towing,and repairs
to Vac-Con truck damaged by sinkhole.

BACKGROUND:
On September 1,2022,CCUA was notified of a sewage backup in the area of 269 Foxridge Rd,Orange
Park. Crews were dispatched and located a backup at the above address. It was discovered that J-
Installations was the responsible party conducting horizontal drilling for AT&T and had bored into the
gravity main allowing soil to infiltrate the pipe and cause a backup. Subsequent repairs were made to
the gravity main in front of 269 Foxridge Rd.
CCUA staff initiated the Emergency Purchase to Recover, Tow and Repair Vac-Con Truck Asset No.
13420 the following Tuesday, September 6, 2022. CCUA's Vac-Con crew were back out to the same
neighborhood to clean out any residual soil in the gravity main down flow from the previous blockage.
The access point was the manhole located in front of 285 Foxridge Rd. As the vehicle approached the
manhole the pavement collapsed,and the vehicle fell into the hole caused by another horizontal boring
into the gravity main. The vehicle immediately came to an abrupt halt. As a result of the fall into the
chole, the vehicle's braking system became damaged.

cz The vehicle was recovered from the hole by ASAP Towing. The vehicle was then towed to an
o authorized service center where a complete DOT inspection was conducted and all damages from the
E incident were identified for repair. Following the truck repairs, an estimated cost will be sent for the
15 Vac-Con system repairs. CCUA is billing J-Installations for all damages and repairs due to their
ce drilling.
RECOMMENDATION:
We respectfully request the Board of Supervisors ratification of the Executive Director's actions under
the Certification of Emergency Purchase and Repairs to CCUA Vac-Con truck damaged by a sinkhole.

I,Jeremy D. Johnston,Executive Director of the Clay County Utility Authority,hereby certify that
the action taken as described below, in conjunction with the attached purchase order and
supporting documentation, is necessary to be taken by the Clay County Utility Authority, an
Independent Special District of the State of Florida, to protect public health, safety and welfare.
CCUA Resolution No. 2013/2014-07, Section 6, Paragraph B, authorizes such emergency
purchases.

Summary
On Tuesday, September 6, 2022, CCUA's Vac-Con crew were at the Foxridge neighborhood to
clean out any soil in a gravity sewer main. The Vac-Con crew was operating in an area near a
previous underground utility strike on September lst caused by a contractor installing
telecommunication cable via horizontal drills. The access point was the manhole located in front
of 285 Foxridge Rd. As the vehicle approached the manhole the pavement collapsed, and the
vehicle fell into the subsurface cavity. The cavity was created from the previous subsurface gravity
sewer main strike that allowed soil to flow with moving groundwater into CCUA's gravity sewer
system.
The Vac-Con is an essential vehicle for CCUA's daily operations. The Vac-Con vehicles are used
in the inspection and cleaning of CCUA's gravity sewer systems as well as responding to
Environmental Incidents (e.g. Sanitary Sewer Overflows). Without the Vac-Con, CCUA
operations will have an idle inspection crew as well as limited capabilities to respond to
Environmental Incidents. The limited capabilities for CCUA crews to respond creates a potential
public health risk as well as a regulatory risk on the utility.
I directed staff to complete the purchases and repairs necessary to return the damaged Vac-Con to
active service. My action to complete the purchases and repairs is necessary for the Vac-Con to
return to CCUA crews at full capabilities to protect public health and mitigate potential regulatory
risks.
I also directed staff to provide the contractor responsible for the underground utility strike a Notice
of Potential Claim to recover costs of the repairs to CCUA's utility system and the Vac-Con.
