I, Jeremy D. Johnston, Executive Director of the Clay County Utility Authority,
hereby certify that the action taken as described below, in conjunction with the attached
purchase order and supporting documentation, is necessary to be taken by the Clay
County Utility Authority, an Independent Special District of the State of Florida, to
protect public health, safety and welfare. CCUA Resolution No. 2013/2014-07, Section 6,
Paragraph B, authorizes such emergency purchases.

a On September 12, 2023, the customer at 355 Crossings Boulevard notified CCUA of a
cc water leak in the front of their property. Crews identified a 10-inch water main break
flooding the area and compromising the entrance and parking area. Staff shut the valves
to isolate the damaged pipe and deployed a repair crew. Internal crews and equipment
could complete the water main repair within a day. However, the asphalt repair is
extensive, and CCUA cannot delay reopening the entrance to the business. Therefore,
CCUA must engage an outside contractor to complete the work immediately and reopen
the entrance. Warren Asphalt Inc will rebuild the entrance in a responsive and timely
manner. CCUA's positive previous experience with Warren Asphalt, Inc., is quality
work.

improvements at 355 Crossing Blvd,which property is owned by (insert Owner's Name).

base will be charged$240.85 per ton.
Terms and Conditions
Certificates of Insurance Supplied upon request.
Warren Asphalt Inc.will not be responsible for pavement defects due to existing base installed by others.
Customer responsible for all buried utilities,including calling for locates.
Pricing based on one mobilization. Additional mobilizations due to site not ready will be an additional charge of$1,500.00 each.
Proposal based on normal Non-Union wages.
Unit prices do not include: Independent Testing,Maintenance of Traffic,Prime Coat,Provision of Traffic Officers,Bonds,Utility location or adjustment,Manhole
or Water Valve adjustment.
Customer repsonsible for all required permits.
Warren Asphalt does not guarantee against ponding,or drainage of water.During Hot weather New Asphalt can be damaged by Vehicles and Equipment.No
warranty is stated or implied.
All proposed quantities are estimated,any change in actual quantities will be adjusted by the unit price multiplied by the actual quantities of work performed.
Warren Asphalt will not be responsible for any damage to underground utilities caused by the normal procedure of our work.

PROPOSAL/CONTRACT IS CONTINGENT ONLY UPON CREDIT APPROVAL BY WARREN ASPHALT,INC.TERMS ARE LISTED ABOVE. ANY PAYMENTS NOT RECEIVED
BY THAT TIME SHALL BEAR INTEREST AT THE RATE OF 1.5%PER MONTH,AND YOU ALSO AGREE TO PAY WARREN ASPHALT,INC.COST AND EXPENSES OF
COLLECTION,INCLUDING ATTORYNEYS FEES WHETHER OR NOT A SUIT IS FILED. ALL LEGAL ACTION WILL TAKE PLACE IN JACKSONVILLE,FLORIDA. NO
RETAINAGE IS TO BE WITHHELD FROM PAYMENTS DUE FROM THIS CONTRACT. ALL PAYMENTS DUE SHALL BE MADE AT THE OFFICE OF WARREN ASPHALT,

WE HEREBY PURPOSE to furnish Material and labor-complete in accordance with above specifications,for the sum below. All material is guaranteed to be
specified. All work to be complete in a manner according to standard practices. Any alteration or deviation from above specifications involving extra costs will
be executed only upon written orders,and will become an extra charge over and above proposal. After Thirty days,Quote expires and is subject to revision. All
agreements contingent upon strikes,accidents,or delays beyond our control. Owner to carry all necessary insurance. Our workers are fully covered by Workers
Compensation.
