Approval of Addendum No.1, to HealthSource Solutions Service Agreement for Wellright Portal

BACKGROUND:
Staff requests approval of addendum No. 1, to Service Agreement for the Wellright Portal,by and
between HealthSource Solutions, LLC and CCUA to extend the term of the agreement from
December 31, 2023, to December 31, 2027.
Employee wellness is essential to the success of Clay County Utility Authority "CCUA". The
CCUA Employee Wellbeing program focuses on helping employees influence their health,quality
of life, and mental wellbeing. As such, CCUA partnered with WellRight in January 2020 to offer
employees a wellness platform that supports employees in their wellness journey.
January 2023 staff was notified that all WellRight clients with less than 500 employee participants
would be managed by a new Partner Network Program, HealthSource Solutions "HSS". At that
time staff recommended, and the Board approved, continuing the relationship with HSS for
continuity purposes.
Staff has been pleased with the services provided by HSS and is requesting to renew the contract
for a period of 3 years 2024-2027 at an annual price not to exceed $16,000 (service agreement
attached).
BUDGET:
Staff budgeted $50,000 for FY 2023/2024 for all employee wellness related fees and initiatives.
RECOMMENDATION:
Staff respectfully requests the Board of Supervisors approval of addendum No. 1 to HealthSource
Solutions Service Agreement for the Wellright Portal.

Summary
By submitting this Order Form, Company agrees to pay HealthSource Solutions monthly with NET 30 terms.
Late payments will be assessed a 3% late fee.
This Service Contract,together with the HealthSource Service Agreement, form a binding agreement between
HEALTHSOURCE and CLIENT. The representative identified below acknowledges they are authorized to enter into
this Agreement.
